● Doctor of Optometry (OD) degree and active state license.

Office

Full-scope primary care private practice in Clarksburg, WV. Sublease agreement with Eye Mart
Express. State of the art equipment: California edition Optomap, Topcon OCT, Octopus Visual Field,
Olleyes Visual Field, FDT Visual Field (used to screen), RetEval handheld VEP, iCare, RapidDx,
Hysteresis testing, and Konan Medical ColorDx CCTHD. In the exam room there are large TVs to
show patients testing results. EMR is Crystal Practice Management. Two pre-test rooms, 4 exam
lanes, 10 employees. Office open Monday thru Friday 8-5, Saturday 8-2:30. Flexible scheduling,
family-oriented group. Currently has two doctors- one part-time/owner, one full time employee
